Urogynecology and Pelvic Floor

Suffering from incontinence, pelvic pain, organ prolapse, or postpartum changes? While there are surgical and interventional methods for treating these conditions, many women prefer to seek treatment in the outpatient rehabilitation setting first. Start your recovery with a physical therapist certified in women’s health.

Incontinence

It’s estimated that one in four women over the age of 40 experience some level of urinary incontinence. This common health issue, which many women find embarrassing to bring up (even with their doctor!), can be treated. Pelvic floor rehabilitation experts with Davis Hospital and Medical Center treat incontinence through manual therapy, biofeedback, electrical stimulation, and by instructing patients in therapeutic exercises.

Treatment is available for other pelvic floor issues (some of which may be related to urinary incontinence):

  • Bowel Incontinence
  • Constipation
  • Organ Prolapse
  • Postpartum Pain
  • Pelvic Pain/Vulvodynia
  • Coccyx (Tailbone) Pain

Hysterectomy

Hysterectomy, the surgical removal of the uterus, is the second most common female surgery in the U.S. In fact, the procedure is so common that one in every three women has a hysterectomy before her 60th birthday. A hysterectomy may be performed for a number of reasons: fibroid tumors, pelvic pain, gynecologic cancer, severe endometriosis, heavy uterine bleeding, and uterine or vaginal prolapse.

Many gynecologists with Davis Hospital and Medical Center use the da Vinci® Surgical System to perform this procedure in a minimally invasive fashion. The procedure involves very little pain and scarring.

Treatment of Endometriosis

The endometrium is a special tissue that lines the uterus. In women with endometriosis, this tissue grows outside of the uterus in other areas of the abdominal cavity. Endometriosis can cause pain in the lower back, pelvis, and lower abdomen – especially during menstruation when the endometrial lining is shed.

Symptoms can vary greatly from mild to severe; some women may experience no symptoms whatsoever. Common symptoms include painful periods, pain during sex, painful urination or bowel movements, bleeding between periods, and more. The gynecologists with Davis Hospital and Medical Center may treat endometriosis through hormonal therapy or surgery.

Urinary Tract Infection (UTI)

Every year, health care providers see 8.1 million urinary tract infections. These infections are much more common in women than men; in fact, a woman’s lifetime risk of having a UTI is greater than 50 percent. Also, the risk of having a UTI increases in women who have had a prior infection.

While most urinary tract infections are not serious, treatment is necessary, as UTIs can cause kidney infection and other health problems. If you’re experiencing a frequent and intense urge to urinate and/or have a painful or burning sensation when urinating, let your primary care provider know, as these symptoms could be signs of a UTI. Most infections can be easily cleared up through the use of antibiotics.

Pelvic Inflammatory Disease

Pelvic inflammatory disease (PID) is a complication caused by some STDs, such as gonorrhea and chlamydia. Risk factors for developing PID include having unprotected sex, having sex with multiple partners, and using an intrauterine device (IUD) for birth control. In women with pelvic inflammatory disease, bacteria spread from the vagina to the uterus and upper genital tract. Symptoms of PID include lower abdominal pain, fever, pain during sex, bleeding between periods, and burning sensation or pain during urination. Some women may have PID and experience no symptoms at all.

Birth Control Options

Women today have more birth control options than ever before. As new methods continue to reach the market, many women find that it’s increasingly difficult to research and evaluate every single contraceptive. Your doctor at Davis Hospital and Medical Center is here to help.

Your physician should take the time to discuss birth control options as they pertain to you. By discussing your goals, lifestyle, and health, your doctor can help you work through the decision process. Birth control options may include:

  • Barrier methods, such as the male and female condom, contraceptive sponge, and diaphragm
  • Hormonal methods, such as the combination pill, mini-pill, Ortho Evra, NuvaRing, or Depo-Provera
  • Intrauterine Devices, such as the copper IUD and the hormonal IUD
  • Permanent procedures, such as tubal ligation and Essure

Diagnosis and Treatment of Sexually Transmitted Diseases

Though education and awareness has likely prevented many cases of sexually transmitted infection and disease, STDs are still a major health issue for many sexually active women. Treatments may be available for bacterial vaginosis, chlamydia, genital HPV infection, genital herpes, gonorrhea, pelvic inflammatory disease, syphilis, trichomoniasis, HIV, and other diseases.

Getting a diagnosis and beginning treatment as soon as possible is critical for patients with a sexually transmitted disease. Some STDs can lead to a “silent” infection (one without symptoms), which may cause serious health issues, such as infertility. Some STDs, such as syphilis, can cause serious complications for a pregnant woman and her unborn child, as well.

It’s important for sexually active women – especially those who have multiple partners – to get regular STD testing. Annual testing and awareness of common symptoms may help reduce the likelihood of chronic infection and long-term sexual health problems.

Treatment of Menopausal Symptoms

Menopause, which typically occurs during a woman’s late 40s or early 50s, can cause many symptoms, including hot flashes, decreased libido, anxiety, mood swings, urinary urge incontinence, hot flashes, fatigue, vaginal dryness, and more. Physicians at Davis Hospital and Medical Center can help with these symptoms (and others). As your body goes through these changes, talk to your doctor about what can be done to minimize symptoms and discomfort.

Hormone Replacement Therapy

After menopause, many women choose to undergo hormone replacement therapy (HRT) to replace estrogen. HRT may provide relief from certain menopause symptoms, such as hot flashes and vaginal dryness. HRT is also believed to provide protection against osteoporosis. Your doctor at Davis Hospital and Medical Center can discuss the potential benefits and risks of hormone replacement therapy with you. Hormone replacement therapy may be administered via pill, gel, cream, topical patch, spray, or vaginal ring.

da Vinci® Robotic Surgery

The da Vinci® Surgical System is one of the most advanced surgical tools in the United States. With da Vinci®, physicians are able to perform complex procedures with greater control than is possible with open surgery or traditional laparoscopy. During the procedure, the surgeon sits at an operating station just a few feet away from the robot. Working with highly sensitive controls, the surgeon can manipulate the robot’s four arms – one of which includes a high-definition endoscopic camera. The tremor-resistant controls, enhanced by a full 360-degree range of motion, allow surgery to be performed with the following potential benefits:

  • Shorter hospital stay
  • Significantly less pain
  • Smaller incisions
  • Less blood loss
  • Faster recovery
  • Faster return to work and normal activities
  • In many cases, better clinical outcomes
